Parent/Guardian Information
Please fill out the information below. All individuals listed as emergency contacts will be given permission to pick up camper.
List any other people who have permission to pick up the camper. If someone is not on this list, and the camp is not notified, the camper will not be allowed to leave with that person. All people picking up must show I.D. at check-out table.
I, the undersigned parent or legal guardian, hereby give permission for my child, to participate in the First United Methodist Church Day Camp (also known as CAMP LIGHT). In consideration of my child being allowed to participate, I assume all risks of injury, illness, accident, or damage that may arise from my child’s participation in camp activities, including but not limited to games, sports, crafts, outdoor play. I certify that my child is physically fit and has no medical conditions that would prevent safe participation. I release, waive, and discharge First United Methodist Church (FUMC) of Murray, its pastors, staff, volunteers, directors, officers, agents, and representatives from any and all liability for any injury, illness, damage, or loss that my child may suffer, whether caused by negligence or otherwise. I agree to indemnify and hold harmless FUMC and its staff/volunteers from any claims, lawsuits, or expenses (including attorney fees) arising from my child’s participation. I consent that, in the event my child is injured or becomes ill while participating in camp activities and I cannot be reached, reasonable medical care and treatment may be administered to my child as deemed necessary by a licensed physician or registered nurse. I further consent for Murray First UMC First Aid personnel to administer any prescribed medication according to written instructions. I agree to pay for any medical treatment not covered by insurance and to be held financially accountable for any damage purposely done to Murray First UMC property or equipment by my child. This waiver shall be construed as broadly as permitted by law. I have read and fully understand this release and sign it voluntarily.
